Title: Community Access Worker - Part-time
Works one-to-one with a selected group of clients and their person-centered support team, to provide services in accordance with individual’s Plan of Care (POC) that promotes independent living skills and development of community connections or natural supports.
Must have at least one year of experience in the field of intellectual or developmental disabilities and have/will have completed a department approved training program within one (1) year of application while providing Community Access services under the direct supervision of a community access specialist. Meet the personnel and training established in Section 3 of the Kentucky Administrative Code.
